OFFLINE DONATION COLLECTION FORM

Download our Donation Collection Form for collecting cash and check donations for Pet Walk.  Send this form, along with your donations by June 12, 2024 to:

Once we have received these donations, we will confirm them and you will see them appear on your personal fundraising webpage.  In order to confirm your gifts are received before the event, all offline donations must be received by June 12. 

**If you receive funds after June 12, please turn them in at the donation & prize pick-up tent on June 15.  Please remember to print and bring along the form with the donations you’ve collected so that we can track them properly.  

OFFLINE REGISTRATION FORM

For those who aren't able to register online, download our Offline Registration Form and fill one out for each person registering for the walk.  Send this form, along with your registration fee by June 10, 2024 to:

After June 10, you must register at the registration tent starting at 8 a.m. on Saturday, June 15 at Veterans Park for day-of registration prices.
